Required Application Documents

CPT is an official authorization, it is vital all documentation is complete and accurate.

Before applying for CPT authorization, you must compile all of the following:

• CPT Online Workshop Completion Email
• Academic/Faculty Advisor Recommendation Form for CPT Links to an external site.
• Detailed training description from your company/employer
• Photocopy of your current I-20
• Print out of your Most Recent I-94 or photocopy of paper I-94
• Your unofficial transcript from Wolverine Access showing CPT course enrollment

 

If you will have multiple employers during the same term, please complete a separate application for each employer/company.
